Community Access Program Fact Sheet
This document summarizes the accomplishments of the Community Access Program of Miami-Dade County (CAP). CAP is a collaborative that works on increasing access to healthcare for the unisured through coordination of services and disease management. It is funded by a Health Resources Services Administration (HRSA), the Health Foundation of South Florida and the Jackson Memorial Foundation.
