CHW Rochelle
CHW Rochelle was conducting outreach at a Laundromat in Little Haiti where she met a woman who was hesitant to speak with her, but was very curious about her work. Rochelle began a conversation with the woman and soon found out the woman had three children with no health care insurance. When Rochelle asked her if she had applied for KidCare, the young woman told Rochelle that she had tried applying before but the application was not processed because it was incomplete. The woman had had such a hard time filling out the application that she never reapplied. Rochelle quickly took all her information and referred her to an HN at Economic Opportunity Family Health Center. The HN met with her and reapplied for KidCare and within a month all of the woman's children received coverage under the KidCare program.
CHW Tracy
A young man Tracy has been working with has suffered from diabetes for many years, but he seldom received treatment. He was in tremendous pain from the disease and had not seen a doctor in over a year. He came to Tracy for guidance on many occasions. Finally, Tracy asked him why he had not gone to the doctor's office. Max (whose real name has been changed to protect his identity) replied that he was afraid of asking dumb questions and he did not want to be criticized by his doctors. Tracy was determined to help. She drew on her experience advocating for her grandmother's declining health, and she set to work on increasing Max's confidence. She spent extra time with Max to make him comfortable advocating for his rights. She role played with him, pretending to be a doctor consulting with him about his diabetes complications. They went through scenarios until Max felt comfortable asking questions about what he needed to know. He gained confidence and began to work with the doctors to whom Tracy had referred him. Currently he is reporting that he is healthier and no longer in pain. Tracy has stated that Max's current medical home is Jefferson Reeves and that he has a good relationship with his doctor.
